A Chronic Disease Management (CDM) Plan is arranged by your GP and enables you to work with a Dietitian manage chronic diseases such as asthma, cancer, cardiovascular disease, diabetes, musculoskeletal conditions or stroke.
To be eligible for a Medicare rebate, you must be referred from a GP on a Chronic Disease Management (CDM) Plan. This entitles you to a maximum of 5 Allied Health Services (i.e. Dietitian, Physiotherapist, Podiatrist, etc) per Calendar year.
The Medicare rebate for Dietetics for those with a CDM plan is $53.80, which will be processed, if you have linked your bank account details to your online Medicare account. Otherwise, you will need to claim with Medicare over the phone.
The total pre-rate cost for all CDM plan consultations is $53.80, whis is payable at the end of your consult. Following your Medicare rebate being processed, your total out-of-pocket costs will be $0.
